Support Co-ordination

We focus on you and we are here to help you design the support you need. We will assist you to access your supports and coordinate them more effectively. We commit to making sure that you get the support you need to help you achieve your goals and aspirations that reflects your individuality, interests, needs and culture.

Our highly qualified and caring Support Coordinators will:

  • Help you to choose and organise your preferred providers for the supports you need
  • Create bookings with your chosen providers
  • Work with you to develop an individualised management plan that addresses your specific needs and goals
  • Help you to build a connection with and participate in your community
  • Regular check-ins with you to make sure you are getting the most out of each service
  • Help you develop the skills to self-manage your supports in the long-term
  • Manage crisis situations on your behalf

Support Connection

Support Coordination

coordination of supports: This support will assist you to build the skills you need to understand, implement and use your Plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community

Specialist Support Coordination

This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.
